	REGISTRATION
	Player Registration Form Also known as the Medical Release Form.
	Administrator Assignment Form
	Player Release and Transfer Form To be used from the First Monday after Thanksgiving through July 31st. Releases & Transfers are not considered complete until the District Commissioner has approved the request in CORIS
	Player Transfer Waiver Form To be used during the player transfer roster freeze period from August 1st to the First Monday after Thanksgiving.
	Cal South Senior Trial Games Card For youth eligibilty clearance only. It allows a youth player to continue to play for his/her curent team while participating in senior trial games. Please see form for further details.
	United States Soccer Federation International Waiver Form

	INSURANCE
	Insurance Claim Questionnaire A copy must be faxed or mailed to the Corporate Office in case of an injury during a Cal South sanctioned event, including League play.
	Liability Insurance Only sanctioned league Presidents may log in to request proof of field liability insurance.

	COACHING EDUCATION
	Coaching License Waiver Application A waiver is not the granting of a license. A candidate must pass the course to receive a license. For more information on eligibility, please click here.
	Youth Module Hosting Form A League, District Head Coach or District Commissioner can sponsor a Youth Module Coaching Course. The Sponsor must fill out a hosting form and send it to the Cal South State office at least 30 days prior to the starting date of the clinic.

	RISK MANAGEMENT
	Request for Live Scan Form Fill out and print 3 copies to take with you to your chosen Live Scan Site
	Applicant Live Scan Sites Links directly to the Department of Justice website to find a location to get your fingerprints taken

	SUPPLY ORDER

	US Youth Soccer Association Travel Requests For teams traveling out of Cal South area or participating in non-Cal South sanctioned events
